Episodic diencephalic hypoperfusion in Kleine-Levin syndrome

Abstract

|
We report a 24 year-old woman presenting with recurrent episodes of hypersomnia, apathy, personality changes, irritability and hyperphagia since February 2001. Brain single photon emission computed tomography (SPECT) was performed during symptomatic and asymptomatic periods, respectively and then subtracted ictal SPECT coregistered to MRI (SISCOM) was done between those two SPECT images. Interictal brain SPECT suggested suspicious hypoperfusion in right mesial temporal region whereas ictal SPECT showed hypoperfusion in bilateral frontal and temporal lobes, and thalami/hypothalami. SISCOM showed significantly decreased perfusion in thalamus/hypothalamus and frontotemporal regions during a symptomatic period.
